Improving community access to mental health services

Our Referral and Access Team assists GPs, health services, community members and others to understand, navigate and access available mental health services.

The Referral and Access Team includes mental health clinicians who are able to assess clinical risk, triage as required and assist people to identify and access the service that best suits their needs including EMPHN commissioned services. 

The team seeks to support referrers and assist clients to access appropriate care quickly. Alternative options and support accessing other services is also provided where appropriate.

  • Referrals to EMPHN's Mental Health Stepped Care service can be made by anyone in the community with the client’s consent (this includes from clients and their families/carers directly). Professionals making referrals should complete the Mental Health Stepped Care referral form in the right hand column.
  • PIR (outer-east) referrals are screened for eligibility and referred to the appropriate mental health support service. Referrals can be made by any member of the community with the consent of the individual wishing to access the service.
