We are an organisation established to increase the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes, and to improve coordination of care to ensure patients receive the right care in the right place at the right time.
We work across an area of 3,956 km2 comprising a population of over 1.5 million people. Cultural diversity across the catchment is high with one in three people born overseas and an equal number of people using a language other than English at home. There is also a prominent Indigenous Australian population of over 5,000 people.
We work across all or part of the 12 local government areas (LGAs) below:
2015/16 was our first year of operation under the Australian Department of Health’s Primary Health Networks Program. Activities have included establishing our legal and organisation structure, consolidating the programs of three Medicare Locals, maintaining continuity of services and preparing for a new role as a commissioner of medical and health care services.
We are now in our second year and look forward to taking big strides forward along with our partners in redesigning and improving our primary health care system to provide better health outcomes, better patient and health provider experiences, and a better and more integrated system overall.
