The Integrated Diabetes Education and Assessment (IDEAS) program provides comprehensive medical and allied health consultations including risk assessment, for people with type 2 diabetes.
IDEAS supports people with diabetes to improve their own health in a community setting instead of in hospital, and reduce the long-term health concerns.
The team provide support and education covering a person's physical health, diet and other lifestyle factors.
The care team is made up of an endocrinologist, diabetes nurse educator, podiatrist and a dietetic to provide individualised care for patients.
