Integrated Diabetes Education and Assessment (IDEAS)

Integrated Diabetes Education and Assessment (IDEAS)

The Integrated Diabetes Education and Assessment (IDEAS) program provides comprehensive medical and allied health consultations including risk assessment, for people with type 2 diabetes.

IDEAS supports people with diabetes to improve their own health in a community setting instead of in hospital, and reduce the long-term health concerns.

The team provide support and education covering a person's physical health, diet and other lifestyle factors.

The care team is made up of an endocrinologist, diabetes nurse educator, podiatrist and a dietetic to provide individualised care for patients.

IDEAS is provided by Carrington Health and its partners Access Health and Community, EACH, Eastern Health and Insprio.
