We have funded Banyule Community Health, healthAbility, EACH, Victorian Aboriginal Health Service and the Eastern Health Yarra Valley Aboriginal Health Team to deliver the Integrated Care program for Aboriginal people in our community.
This service offers:
- coordination of medical support and support to address cultural, financial, social and emotional factors that impact on people’s health and wellbeing,
- outreach support,
- support to access health services,
- transport to medical appointments and financial assistance for specialist visits, in some cases, and
- medical equipment when otherwise difficult to access and when critical to the health and wellbeing of individuals in the program.
Eligibility
People from Aboriginal Communities who have a medical condition that has been or is likely to still be present in 6 months can access this service.
