Transition of Outer East Head to Health consumers to the Yarra Ranges Mental Health & Wellbeing Local Service

As of March 2024, referrals to the outer east Head to Health Hubs provided by Inspiro and healthAbility will cease and new referrals will be redirected to the Yarra Ranges Mental Health & Wellbeing Local Service.

The outer east Head to Health service providers, and the Yarra Ranges Mental Health & Wellbeing Local, are currently working together to plan for clear, timely and well communicated transition options for those consumers who make an informed decision with the support of their current Head to Health provider to transition to the Local service. All existing consumers will be given the opportunity to review their care pathways, the opportunity to learn about their options and will have this information delivered in a manner that best supports their circumstances, along with their families, carers and other service providers.

Outer east Head to Health providers will then work through the individualised transition plans to support continuity of care for those consumers who choose to transition their care to the Yarra Ranges Mental Health & Wellbeing Local. Once a consumer’s supports are completely transitioned, the Yarra Ranges Local will assume responsibility for providing any required treatment, care and support.

For those consumers who choose not to transition their care to the Yarra Ranges Mental Health & Wellbeing Local or are below the age of 26, Head to Health providers will continue care where appropriate or transition consumers to other available and suitable supports.
