Do you have patients who are homebound and remain unvaccinated?

EMPHN are now accepting referrals for in-reach home visits from Health Care agencies and providers, GP’s, and Health communities within the catchment to vaccinate people who cannot leave the home to receive a COVID 19 vaccination.

South Eastern Melbourne (SEMPHN), North West Melbourne Primary Health Network (NWMPHN) and East Melbourne Primary Health Network (EMPHN) have partnered to deliver the vaccination program to homebound clients within their specific regions. The three PHNs will engage with various health agencies and communities within their catchments to assist in identifying those that may be homebound and remain unvaccinated.

The Vulnerable Peoples Homebound Vaccination Program (VPHVP) is an Australian Government initiative to provide vaccinations to the vulnerable populations. The homebound may include but are not limited to the frail aged and elderly, and people living with a disability or a mental health condition which prevents them from leaving their home.

Once the referral has been received, a vaccination provider will contact the homebound person (or nominated contact person) to schedule a time for the in-reach home visit.

All requests must come through the referral link provided to be scheduled for a vaccination.

This is a short-term activity operating from November to the 31st December 2021 (with some dose 2 vaccinations administered in early-mid January).
