Tender opportunity: Enhanced Palliative Care at Home initiative

Eastern Melbourne Primary Health Network (EMPHN) invites tender responses to provide Enhanced Palliative Care at Home.

The purpose of the program is to provide community-based support for people (and their carers) in their last 12 months of life to enable them to remain living at home or in a home-like environment, such as a residential aged care facility, and prevent unnecessary hospitalisation.

The provider may choose a range of service delivery options that will:

  • be supportive (enhance psychosocial wellbeing, physical comfort, decision making)
  • be informative (raise awareness, educate, improve service navigation)
  • provide connections (to family, friends, peers, community, and other services)
  • be practical (assist with daily living)
