Lilydale Integrated Youth Health Hub – draft service model feedback

Eastern Melbourne Primary Health Network (EMPHN) has received funding from the Australian Government to develop an Integrated Youth Health Hub in the Shire of Yarra Ranges. 

The hub will provide young people and their families access to a range of integrated primary health care services including mental health, physical health, drug and alcohol, as well as a range of other community services.

The consultation process thus far has involved a number of stakeholder consultations, and has resulted in the development of a high-level draft service model for the hub.

We are now seeking feedback from stakeholders in relation to the draft service model of the hub in order to refine and finalise it.

We would very much appreciate you taking the time to read the draft service model, and respond to the questions below by Thursday 30 January.

It is anticipated that the hub will commence operations in July/August 2020.
