Nexus Primary Health and partners win tender for diabetes service

This National Diabetes Week, Eastern Melbourne PHN (EMPHN) is delighted to announce Nexus Primary Health is the successful tenderer to provide innovative diabetes diversion services in Whittlesea, and the southern parts of Mitchell and Murrindindi.  

This week, Diabetes Australia’s "It’s About Time" campaign aims to raise awareness about the importance of early detection and early treatment for all types of diabetes.

Nexus Primary Health have been commissioned to provide Type 2 diabetes diversion initiatives that encourage innovative integrated models of care.  

Nexus Primary Health was successful with their Diabetes Diversion Partnership Project (DDPP) submission, an innovative, integrated multidisciplinary community based care model supported by care team health technology.  

The partnership, led by Nexus Primary Health (Nexus), includes DPV Health, Precedence Health Services and Hume Whittlesea Primary Care Partnership (HWPCP), and is based on a medical home model. 

The partnership will work closely with Northern Health, Kilmore and District Hospital and other stakeholders.

This work will build toward the long-term outcome of improving patient outcomes for residents of Whittlesea, Mitchell and Murrindindi and reducing demand on acute services.  

Planning has started and the new diabetes services will open by 1 October 2018.
