The Dementia Directory supports people living with dementia to better access care the care they need to live well in the community for as long as possible.
The Dementia Directory is a directory of local services and support for people living in Melbourne with dementia, as well as their carers and families. It complements existing directories for My Aged Care and NDIS.
Why was the dementia directory developed?
Primary health networks were funded by the Australian Government to develop dementia resources, as a response to the Royal Commission into Aged Care Quality and Safety.
This directory and accompanying information sheet have been developed for people who live in the northern, western and eastern suburbs of Melbourne.
The information sheet is designed to be given to people when they are diagnosed, so they may refer to it later. It includes information about all the key organisations that can help, including Dementia Australia, Carer Gateway, My Aged Care, and the NDIS.
Information sheets are available in English, Italian, Greek, Vietnamese or Simplified Chinese. Download it here.If you are interested in placing an order for these resources, please email firstname.lastname@example.org.
