Care Finders in EMPHN

Eastern Melbourne PHN is releasing a tender to the market on 5 September for new Care Finders to become operational from January 2023 across the entire EMPHN catchment. Care finders will provide specialist and intensive assistance to help people within the care finder target population to understand and access aged care and connect with other relevant supports in the community. This is the first of two tenders in a phased approach.

Care finders will complement, and should not duplicate, My Aged Care. Care finders will specifically target senior Australians who need intensive support to understand and access aged care as well as connect with other relevant supports in the community. This will include people who are not yet receiving aged care services, as well as those who are.

People who are eligible for aged care services and have one or more reasons for requiring intensive support to:

  • Interact with My Aged Care and access aged care services.
  • Access other relevant supports in the community.

VTPHNA Care Finder Market Briefing

The Australian Government is funding Primary Health Networks across Australia to commission the new care finder program. The Victorian PHNs have worked collaboratively to develop consistent contract documents and a common tender response document. A single care finder Market Briefing will therefore be delivered collaboratively by all participating PHNs, online, on Monday 12 September at 1pm.

The market briefing is your opportunity to learn more about the program, the commissioning and contracting process, to build your understanding of PHNs probity obligations and learn about the next steps following the tender closing period on 30 September 2022.
