Name of tender: Mental Health Stepped Care – Outer East 

Description: To provide Mental Health Stepped Care services in the outer-east of EMPHN’s catchment, covering the Local Government Areas of Knox, Maroondah and Yarra Ranges. The Mental Health Stepped Care Model aims to deliver a continuum of primary mental health services within a person-centred, stepped care approach, for people of all ages. Name of tender: headspace - Monash 

Description: Establishment and operation of a new headspace centre in the City of Monash. headspace centres provide integrated healthcare to young people (aged 12-25 years) with mental health and wellbeing concerns. The headspace centre is planned to be operational by mid-2020.

Name of tender: Healthy Ageing Service Response 

Description: The delivery of a Healthy Ageing Service Response to the whole of EMPHN’s catchment with a focus on older persons mental health and wellbeing. The service response is targeted for people aged 65+ years (or 55+ years for Aboriginal and Torres Strait Islander populations) who live in the community or in Residential Aged Care Facilities. This new service response will have a focus on primary and secondary consultation, multidisciplinary care, collaboration and sector capacity building.

Name of tender: Preventative Health Screening Initiatives for Mental Health Consumers 

Description: Deliver an integrated service response to address barriers people with mental health issues face in actively participating in health promotion and prevention programs including: 

  • national screening programs for breast, bowel and cervical cancers
  • access to influenza immunisation 
  • evidence-based smoking cessation programs. 

The integrated service response will be available to consumers with mental health issues across EMPHN’s catchment.

Name of tender: Lilydale Integrated Youth Health Hub 

Description: EMPHN has received funding from the Australian Government to establish an Integrated Youth Health Hub in Lilydale. The Integrated Youth Health Hub will be a single entry point for young people and their families/carers to access a range of primary health care and community services.

Name of tender: Secondary Prevention of Chronic Pain for Healthy Aging

Description: The initial implementation will focus on the preservation of physical function and psychological wellbeing into older age by avoiding chronic pain post an acute event.

Name of tender: Mental health and AOD navigation platform, rebranding and marketing campaign 

Description: Development of an integrated navigation platform (e.g. communication resources and website) for the psychosocial service system, ensuring EMPHN, state-based and NDIS psychosocial services are easy to navigate for consumers, GPs and service providers, with a view to minimising gaps for consumers wishing to access support. This includes re-branding and promotion of EMPHN's Referral and Access Team. The platform must provide information and resources for consumers, carers and providers in the EMPHN region, including development of targeted resources and communication collateral for priority groups within the EMPHN catchment.
