Primary Care Quality Improvement Programs

Eastern Melbourne PHN (EMPHN) invites general practices to apply to participate in any of our Quality Improvements (QI) programs below.

EMPHN will be releasing expressions of interest via an online system (eProcure). This system enables EMPHN to provide a transparent and accountable process in line with Commonwealth guidelines. Registering is free, and easy to do, just follow these steps:

Heart Failure QI Program

Applications open: 2 November 2022

Six month program to enhance GP capacity to manage people with heart failure in primary care, in line with best practice guidelines, relevant public health intiatives and systems.

EMPHN is seeking to recruit four general practices to participate in a QI program and apply a whole of practice approach to enhance their management of patients with heart failure. Through attendance at collaborative workshops and completion of PDSA cycles provided by the HeartPlan in a Box QI Toolkit, general practices will be supported to align their management with recommendations from the National Heart Foundation of Australia and Cardiac Society of Australia and New Zealand: Guidelines for the Prevention, Detection, and Management of Heart Failure in Australia 2018.

All general practices located within the EMPHN catchment are encouraged to apply.

What’s in it for the practice?

  • This activity will support general practices to undertake data-driven quality improvement
  • Incentive payments upon meeting objectives, up to $7,000
  • Greater understanding and support for best-practice clinical management and local care navigation
  • Access to clinical champions to consult with your clinical team and connection to local health practitioners
  • CPD points available for completing relevant GP and Practice Nurse activities
  • Improve heart failure outcomes and experience for patients

Cancer Screening QI Program

Applications open: 3 November 2022

Opportunity for 10 general practices to participate in a QI program to improve patient engagement with cancer screening.

Using a whole of practice approach, practices will focus on increasing engagement with either the National Bowel, Breast or Cervical screening programs, or liver cancer screening, and will embed use of the National Cancer Screening Register (NCSR). 

Practices located in Whittlesea LGA are encouraged to apply.

What’s in it for the practice?

  • Funding of $10,000 will be available to general practices who complete the QI and reporting requirements.
  • Increase in clinician (or their PRODA delegate) utilisation of the NCSR to improve identification of never screened or under screened people and record accuracy.
  • Development of practice teams QI skills and knowledge.
  • Undertake a QI program that will contribute to PIPQI activity requirements.
  • Improve cancer screening rates for your community.

Eye Health QI Program

Applications open: 3 November 2022

Opportunity for 10 general practices to participate in a QI program to improve the eye health of their practice populations.

EMPHN is seeking to recruit ten general practices located in the LGAs of Knox, Whittlesea or Yarra Ranges to participate in a QI program to assist general practices to improve the eye health of their practice populations. Using a whole of practice approach, practices will focus on increasing engagement with local optometrists and ophthalmologists.

What’s in it for the practice?

  • This activity will assist in general practices meeting Practice Incentive Program (PIP) Quality Improvement (QI) activity requirements
  • Incentive payment upon meeting objectives, up to $7,000
  • Greater understanding of the importance of eye health referral pathways and feedback for patients
  • Cost effectiveness on patient health outcomes (feedback loop on outcome of consultation)
  • CPD points available for completing relevant GP and Practice nurse eye health online training
  • Improve eye health outcomes for patients
