People living in and around Wallan and Kinglake will soon benefit from a new innovative after-hours program called the 'Medical Neighborhood' that will be delivered by Nexus Primary Health and funded by Eastern Melbourne Primary Health Network (EMPHN). The Medical Neighborhood is an expanded patient-centered care model where GPs, specialists, hospitals and other health care professionals work together to provide coordinated care for patients.
Senator James Paterson said the initiative will help the Wallan, Kinglake and surrounding communities to prosper.
“This investment in the health of our community takes into account the busy nature of modern life and the need for flexible healthcare,” he said.
EMPHN CEO Robin Whyte said the model links different parts of the health system in the after-hours setting so they work together to provide a more seamless experience for patients.
“Many people find it difficult to access healthcare during normal business hours for a variety of reasons.
“This service helps people access the care they might otherwise go without and provides a one stop shop for integrated healthcare,” she said.
Nexus Primary Health CEO Suzanne Miller said the expansion of services is about more than just extended hours and the service aims to engage people in the community who are facing challenging life circumstances.
“In addition to being open for longer, we will be increasing our outreach visits and our telehealth program with Northern Health to make sure our community can access the healthcare they require, no matter their individual needs,” she said.
Nexus Primary Health will expand their capacity for home visits by GPs and nurses for clients with complex needs, in particular those with disability or in palliative care.
The extended hours will also include physiotherapy, psychology and paediatric services. Nexus Primary Health will employ a Systems Navigator as part of the program to provide a welcoming point of connection for people experiencing disadvantage or disconnection, with a focus on engagement and support to better understand their issues and address health care needs.
