NURSE WATCH
The practice will nominate a nurse to participate in 12 week program course designed by Kim Poyner (Medicoach). This program offers Practice Nurses training, networking and mentoring to strengthen and extend their role with managing complex patients in a telehealth environment. RIGHT CARE = BETTER HEALTH
The practice is assigned a ‘complex health coordination and linkage worker’ to provide integration and care coordination for patients. The practice will nominate a GP, a Practice Nurse and their Practice Manager to participate in the Right Care = Better Health program and workshops.
