EMPHN invites expressions of interest for new program aimed at practice nurses

Eastern Melbourne PHN (EMPHN) is seeking expressions of interest from practice nurses to undertake a series of workshops (health coaching, integrated care, care coordination) and supported mentoring sessions.

The initiative aims to build the skills of primary care nurses to align with the Australian Government’s vision for how care will be delivered under the proposed Health Care Homes model, which will improve management of chronic disease.

A suite of professional development workshops, webinar and supported mentoring sessions have been developed to build the skills of primary care nurses.

Nurses within the our catchment have a diverse scope of practice and are well positioned to address the emerging healthcare challenges.

Building on trusting patient relationships, they have the capacity to integrate and coordinate relevant members of the broader health care team and fill the need as health coaches, educators, chronic care managers and care coordinators.
