Eastern Melbourne PHN (EMPHN) is a Primary Health Network established to increase the efficiency and effectiveness of medical services for patients, particularly those at risk of poor health outcomes, and to improve coordination of care to ensure patients receive the right care, in the right place, at the right time. We work across an area of 3,956 km² comprising a population of more than 1.5 million people across 12 local government areas in east and north east Melbourne.
EMPHN was formed through the consolidation of three Medicare Locals under the Australian Department of Health’s Primary Health Networks Program. Since its first year in operation in 2015-16, EMPHN has transitioned from direct service provision to a commissioning organisation that strategically plans and invests in quality primary healthcare services with the goal to make a difference to the health and wellbeing of the community.
Commissioning requires working with stakeholders and the community to understand their needs and the existing system, co-designing solutions and models to meet needs, and a genuine collaborative approach to working with the sector to implement the changes required.
